Quick Facts

  • Monday – Friday schedule with participants finished by 3pm
  • 3 – 9 patients per day with extended one-on-one time
  • Travel to assisted living and skilled nursing facilities as needed
  • Call coverage 1:4 rotation with experienced RN triage support
  • Team-based care with interdisciplinary meetings twice weekly
  • Family medicine, geriatrics, or primary care experience required
  • Hospital privileges required
  • Board certification required
